repeats in patterns

by Karen Hughes

When a pattern states that it is a repeat of 11 plus 1, I cast on 12 stitches. If I want to enlarge the pattern and make four repeats do I cast on 48 stitches or 44 plus 1 ?

Thank you for your help......Karen
